Chronic kidney disease (CKD) is a health problem affecting millions of people, in Poland, the number of patients is estimated at approximately 4.2 million. At our company, we place great emphasis on bringing innovative products related to the treatment of CKD to market for several key reasons.

Currently, despite the decline in diagnosis due to the pandemic, the number of deaths due to HF reaches almost 150,000 annually. This poses a challenge for healthcare systems, especially in the context of an aging population and increasing life expectancy.
In response to these challenges, proposed strategies to improve the care of patients with HF include dedicated care models such as case management, disease management, and clinical pathway management. Early diagnosis is crucial for effective long-term care, bringing lower costs and reducing the risk of complications. Patients with HF require constant medical care, access to modern therapies, rehabilitation, and palliative care.
Technological development enables effective management of HF using telemedicine solutions and monitoring. There is a need to create an integrated model of care for patients with HF in Poland, including the continuity of outpatient and hospital care. A key element is the cooperation of multidisciplinary teams and improved communication between patients and doctors.
